  4. Currently Valued Loss runs for the last 5 years
  5. If real property coverage is desired, the following information is required on each and every building: square footage; construction; occupancy; approximate age; and if older than 25 years, what has been updated and when (plumbing, electric, roof, structure). This Statement of Value (SOV) is preferred.
  6. Staff employment application & Camper enrollment form
  7. Map of camp (does not have to be a scale map)
  8. Camp activities safety manual and/or procedures including all written material on sex abuse prevention, screening, monitoring and reporting
  9. Winter Freeze Survey
  10. Current and target premium
  11.  Signed Brokerage Agreement W9 Form, E&O cert and license
